The only other character known to have the third eye in the MCU is Sinister Strange, who got the third eye after using the darkhold of his universe to enter into different multiverses to kill every other multiversal version of himself. 

This theory, when thought of along with why Doctor Strange has a third eye, can be directly related to using the Darkhold, and many MCU theorists have related this with the possibility that Doctor Strange of universe 616 might be in danger due to him getting the third eye. However, things might not be the same way with Doctor Strange.

However, the MCU’s depiction of why Doctor Strange has 3rd eye can also be very different from the comic version. Doctor Strange gained the third eye after using the Darkhold, which is known for corrupting anyone after using it despite how strong that individual is.

In the MCU, the Darkhold pulled Agatha Harkness and Scarlet Witch into villainous beings, and despite Doctor Strange limiting his exposure to the Darkhold, he grew a third eye.

So is Doctor Strange’s third eye an ill omen? Some theories contradict this saying as the reasons why Doctor Strange’s relation with Darkhold may not be as ominous as Scarlet Witch and Agatha Harkness because Scarlet Witch and Agatha Harkness actively looked for Darkhold for their own selfish purpose, which in turn turned their fingers dark, but beyond that nothing else happened. 

In a different scenario, Doctor Strange only used the Darkhold as the last resort in order to stop the Scarlet Witch.  

This is why Doctor Strange having an ominous third eye cannot be justified by the Scarlet-Agatha duo’s involvement with the Darkhold.

Besides, in the 2016 movie, Ancient One’s mention of opening Doctor Strange’s third eye also suggests that Doctor Strange having the third eye can be more beneficial than ominous, and the fact that Doctor Strange can still control his third eye in the post-credits scene of Doctor Strange In The Multiverse Of Madness indicates that it is not much of a scary deal.
